Assessor Nutrition (Temp)

SUMMARY

Initiates, pre-qualifies, and processes intake applications for the Meals on Wheels (home-delivered meals) program. Act as a liaison between referring agencies and potential clients.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Submit accurate, thorough assessments promptly and maintain strict confidentiality.

  • Conduct 701A (Activities of Daily Living) and 701S (Nutritional Risk and Screening) assessments in alignment with DOEA standards and guidelines.

  • Interview clients to assess health, functional ability, nutritional risk, and overall service needs.

  • Communicate effectively with clients, caregivers, and DOEA representatives as needed.

  • Provide follow-up information when required and ensure compliance with state and federal regulations.

OTHER SKILLS AND ABILITIES

  • Prior experience conducting elder care assessments or working in a social services or healthcare field.

  • Knowledge of Florida Department of Elder Affairs programs, particularly 701A and 701S assessments.

  • Strong communication skills with a compassionate, client-centered approach.

  • Highly organized with the ability to manage multiple assessments independently.

  • Must have completed or be willing to complete DOEA 701A and 701S assessment training

  • Familiarity with electronic assessment tools is a plus.

SUPERVISORY RESPONSIBILITIES

This job has no supervisory responsibilities.

EDUCATION and/or EXPERIENCE

Must have completed or be willing to complete DOEA 701A and 701S assessment training. Ability to work independently as a 1099 subcontractor with flexible scheduling. Reliable transportation to meet clients in various locations. Flexibility and availability to travel within the Pinellas County.
