Intake Specialist-Nutrition

SUMMARY

Initiates, pre-qualifies, and processes intake applications for the Meals on Wheels (home-delivered meals) program. Act as a liaison between referring agencies and potential clients.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Determines eligibility for home-delivered meals (meals on wheels) program
  • Refers ineligible clients to the congregate dining program
  • Refers clients to the Aging and Disability Resource Center for needs outside of nutrition
  • Provides alternate suggestions for underage clients who don’t qualify for the Meals on Wheels program by referring from the current OAA list of services
  • Conducts a 701S phone assessment, creates an eCIRTS client file
  • Places clients on the MOW waitlist
  • Explains other nutrition programs OAA-funded program, Private Pay, and any other third party-funded meal program
  • Completes 701A in-person assessments as needed.
  • Completes 701S reassessments
  • Maintains 701S appointment schedule and appointment slots
  • Assist with terminating waitlist clients as appropriate
  • Maintains client confidentiality and follows HIPAA policy

OTHER SKILLS AND ABILITIES

  • Knowledge of Neighborly programs, policies & procedures.

  • Knowledge of the special needs of older adults
  • Basic computer skills, data entry, record keeping, and clerical office procedures.
  • Basic knowledge of Office 365
  • Effective oral and written communications.
  • Work efficiently under pressure with deadlines
  • Work effectively with community/statewide agencies
  • Maintain department, agency and client confidentiality
  • DOEA 701B and eCIRTS training required for input of assessments and reporting

SUPERVISORY RESPONSIBILITIES

This job has no supervisory responsibilities.

EDUCATION and/or EXPERIENCE

High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
